Leading experts on quality management in health care will cover more and more important - how to ensure favorable results in clinical patients transition to another or home environment.

Transitions in care can be broadly defined as implemented through the continuum of care, such as within a health care organization, in all environments (for example acute post-acute) and in a community practice or population.
The gaps in the quality and safety of care transitions have been documented in several studies.
